Preschool Application Packet

Preschool Tuition Assistance 2026-2027

 

 

  • Please fill out Job and Family Services (JFS) form 07200 (Child Care Assistance Programs Sections)

  • Please fill out the Easy Care & Education Services Release of Info form DCY01115 (This forms allows JFS to communicate with Amanda-Clearcreek Preschool)

  • Please fill out Early Care and Education Services Selection form

  • Please provide 30 days of household income 

  • Please send all forms along with documentation to Ms. Dwyer via email tdwyer@amanda.k12.oh.us or set up a time to drop off at the Preschool Office (740) 969-7254 x 4164

  • Amanda-Clearcreek Preschool will submit your application to JFS

  • JFS will notify you if they need any additional information

  • JFS will notify you by mail if you have been approved for assistance

  • JFS does not determine your tuition cost.  Amanda-Clearcreek will notify you of your tuition cost
